Additional Specialist Support
Housing Outcomes known as Supported Independent Living (SIL), Specialist Disability Accommodation (SDA), Individualised Living Options (ILO) or Medium-Term Accommodation (MTA) are all difficult to navigate without the support of an experienced Support Coordinator.
Supported Independent Living (SIL)
Our Team has extensive experience in assisting Participants with complex housing support requirements. We can assist you to apply for appropriate funding for your support needs, connect with support providers that fit your situation and monitor and maintain appropriate ongoing support.
Hospital Discharge
A Hospital discharge can be very overwhelming for Participants and families as they are often new to the NDIS and may be coming home for the first time. Having the right Support Coordinator with experience with Hospital discharge will be vital to making your homing coming that much easier.
Your Support Coordinator will work closely with your Hospital Social Worker, Occupational Therapist, Physiotherapist and Speech Therapist to ensure you have all your Assistive Technology and supports appropriately set up and in place before you can be safely discharged from hospital.
Your Support Coordinator will hold a face-to-face meeting with you and your nominated Support person at the hospital to discuss your NDIS plan, budget, support requirements and to ensure you have choice and control with decisions. This will be vital as you will require an efficient team to work closely with yourself and your nominated Supported person to guarantee you are connected with your selected services before discharge.

Assist Participants, Guardians and providers to navigate the legislation and connect with Behaviour Clinicians to work through supports where restrictive practice may be involved.

We provide supports for children ages 7yrs – 18yrs for Agency Managed Participants.
We provide supports for children ages 0yrs – 6yrs for Plan Managed Participants – ONLY.
