Specialist Support Coordination is a higher level of support coordination. This level of support is for people whose situations are more complex and who need specialist support.
Specialist Support Coordination is time limited and focuses on overcoming significant barriers preventing access to service delivery. We can assist if Participants are at a high-risk level by reducing complexities within the support environment.
The desired outcome of Specialist Support Coordination is that complexities and barriers affecting a person’s capacity and ability to implement their plan are reduced.
What is the difference between a Support Coordinator and a Specialist Support Coordinator?
