NDIS SUPPORT COORDINATION
Reality Support Services provide services for both Level 2 Support coordination and Level 3 Specialist Support Coordination.

If you are experiencing difficulties with sourcing and coordinating appropriate supports to implement your NDIS plan, support coordination services may be included in your NDIS plan to help you overcome this barrier.
Support Coordinators can help you find mainstream and other disability services that are right for you.
There are three levels of support coordination that can be included in your plan. These are Support Connection, Coordination of Supports – Level 2, and Specialist Support Coordination – Level 3.
To access support coordination services, you will need to have stated funding for support coordination in your NDIS plan.
If you don’t have this funding in your Plan, give us a call and we will discuss how we may be able to assist.
