Support Coordination is suitable if customers receive multiple or complex supports from a range of providers. Support Coordination will support you to understand and implement the funded supports in your plan and link you to community, mainstream and other government services. A Support Coordinator will focus on supporting you to build skills and connect you to providers.

Once you are receiving services, we will assist you to negotiate with providers about what they will offer you and how much it will cost out of your plan. Support coordinators will ensure service agreements and service bookings are completed. They will help build your ability to exercise choice and control, to coordinate supports and access your local community.

• Ensure services are provided in accordance with your needs and preferences
• Negotiate and develop service agreements between you and your providers
• Monitoring your funds, supports and progress towards your goals.
• Assisting you to recruit your own staff and learn the skills to self-manage your NDIS plan.
• Visit you at home or at a place convenient to you
• Liaising with government agencies and the community on your behalf if required
• Connecting you into work, study and / or other communities based on your personal’s goals
• Working with you to develop your skills, capacity and resilience
• Monitoring your NDIS plan and working with you to review your goals prior to the planning meeting and subsequent reviews
